Phase 8: Reevaluation

Review the previous session and current needs before deciding what to do next.

Awaiting qualified clinical review. Educational reference; not a protocol, formal training, supervision, or individualized treatment.

Suggested use: preparation and shared discussion. No qualified clinical review date has been recorded.

Phase 8: Reevaluation: Ask about the interval: What has happened since last time?; How was daily functioning?; Were supports useful?. Revisit the target: Check its current response; Use relevant ratings when indicated; Notice new or unresolved material. Review today’s needs: Current priorities and safety; Present connection and resources; Readiness for the proposed pace. Choose together: Resume, prepare, or revise; Explain the next step; Document the clinical rationale

Why this matters

Ask about target response, new material, daily functioning, support use, and safety; decide together whether to resume, prepare further, or revise the plan.

Client-facing explanation

We check what has changed and what still needs attention. You do not need to have improved to give a useful answer.

Clinician reference

What have you noticed since we last met? How does the previous target feel now? What needs attention today?

General sequence and discussion checklist

Optional discussion aid. Selections clear when you leave; this does not assess clinical readiness.

Practical example / documentation consideration

Documentation considerations: Record current target status, relevant ratings, functional changes, safety review, and updated plan.
